Mumbai: A team led by Dr Hemant Patel, (General And Laparoscopic Surgeon), Physician Dr Urvi Maheshwari, and Intensivist Dr Neha Patil at Zynova Shalby Hospital successfully treated a 60-year-old Bangladeshi man with the superior mesenteric artery (SMA) thrombosis. A rare disease that involves small intestines. The patient who had complaints such as abdominal pain and respiratory distress due to this condition is symptom-free now and eager to fly back home.
Mr. Mohammad Monirujjaman, a 60-year-old from Bangladesh, previously battled cancer of the intestine and liver, for which he underwent surgical procedures. To his dismay, his condition deteriorated significantly when he began experiencing severe abdominal pain. he went to multiple doctors and ultimately came to Zynova Shalby Hospital.
Dr Hemant Patel, General and Laparoscopic Surgeon at Zynova Shalby Hospital Mumbai said, “Patient was admitted on the day of Diwali afternoon 31 October 2024 with severe pain in the abdomen and respiratory distress. He was then intubated in casualty and shifted to the ICU. His abdomen and chest scan was done with necessary blood reports which revealed a complete small bowel gangrene. It was superior mesenteric artery thrombosis (SMA), a fatal syndrome that occurs when the artery supplies blood to the small intestine and blocks the colon. His main artery which supplies blood to the small intestine was choked. 5 in 1,00,000 people suffer from this condition. Only the 30 cm proximal loop of the intestine was viable and the rest of the small intestine was gangrenous, except the proximal 30 cm with 2 liters of pus in abdomen. This patient also had high creatinine and infection in the blood and was given antibiotics. We decided to remove the entire small intestine leaving a 30 cm loop of the intestine. Laparotomy with near total small bowel resection with proximal jejunostomy was done.”
Dr Patel added, “We took out part of the small viable intestine through separate cuts on the abdomen, and put the stool bag on it. Since the intestine was gangrenous, removing the clot would have solved the problem. Hence, decided to remove the small intestine which took 4 hours to perform surgery. Post-surgery, the patient was shifted to the ICU. He was on a ventilator for 7 days. We took the patient off the ventilator on 7/11/ 2024 and shifted him to the ward on 18/11/2024 and he traveled to Bangladesh. The patient and his son are now in constant touch with us via phone calls and WhatsApp. He is much better now and is doing his daily activities with ease. Not treating him correctly and promptly could have led to loss of life.”
